Please note the instructions below for completing the registration form. You must complete/answer all questions. Your child will not be considered registered until the required documents have been submitted to your child's school.
Once the pre-registration form is complete, with the required documents, you will receive a call from the school that your child is zoned to attend. You will be asked to come in and complete the Dial 4 Screening and finish the registration process. Failure to complete either of these will result in your child not being registered/enrolled for 4k.
